10 Things That Matter in Healthcare at Home · Every Monday

Week of October 12, 2026 · Issue #022 Curated by Sumit Arora
A typo on a renewal form can cost you your Medicare billing. Check the paperwork before you check the dashboard.
The Monday Brief

CMS is giving home health more visibility into denials while its contractors are revoking enrollment over clerical mistakes. A denial dashboard will not save you from a revocation, so every CMS filing needs a pre-submission check.

Payers and regulators are using AI to spot billing irregularities and deny claims faster. That risk is built upstream, at coding and visit documentation, which is where your records need to hold before a claim ever goes out.

And Utah just showed what responsible AI in healthcare can look like: real clinical pilots, physician oversight, and independent third-party evaluation. Expect other states to borrow the model.

01 Regulatory / Payment Alert
Utah Expands Its AI Regulatory Sandbox With New Clinical Pilots
ALL MODERATE

What happened: Utah expanded its AI regulatory sandbox with new clinical pilots in prescription renewals, dermatology, and physical therapy, adding stronger physician oversight and independent third-party evaluation.

Why it matters: This is the first working template for how states may regulate clinical AI, and its two pillars, oversight and independent evaluation, are the standard every AI vendor you buy from will eventually be held to.
What to do this week: Ask any AI vendor you use or are evaluating how their outputs are independently checked, because that question is about to become a regulatory one.
02 Compliance Watch
New Rules Coming for How Agencies Use Flexible and Gig Workforces
ALL HIGH PRIORITY

What happened: New regulations are being introduced on how healthcare organizations classify and engage flexible and gig workers.

Why it matters: If your PRN or contractor model leans on classifications that will not survive the new rules, your labor cost and liability can change overnight.
What to do this week: List every clinician you pay as a contractor or per-visit, and confirm the classification would hold up if someone outside your agency reviewed it.
03 Documentation Issue
Hospice Loses Medicare Billing Privileges Over a Technical Error on a Renewal
HOSPICE HIGH PRIORITY

What happened: A hospice had its Medicare enrollment and billing privileges revoked over a technical error on a routine renewal application.

Why it matters: Revocation does not slow your cash flow, it stops it, and the error that triggered it here was clerical, not clinical.
What to do this week: Assign a second person to review every Medicare and Medicaid enrollment or renewal filing line by line before it is submitted.
04 Payer Update
MACs Now Post Quarterly TPE Results as Enforcement Stays Inconsistent
ALL HIGH PRIORITY

What happened: Home health and hospice providers are reporting inconsistent and aggressive audit and enrollment enforcement from MACs, and MACs are now required to post quarterly Targeted Probe and Educate results on their websites.

Why it matters: When enforcement varies by contractor, the published TPE data is your only window into what your MAC is actually targeting.
What to do this week: Pull your MAC's latest posted TPE results and compare their focus areas against your own top billed services.
05 TrueTime Insight
AI Is Reviewing Your Claims. Something Should Review Them First
ALL MODERATE

Payers and regulators are using AI to flag billing irregularities before a human ever opens the file. TrueTime Chart Review checks each chart against coverage criteria, and every finding is checked again by an independent second AI reviewer before it reaches you, the same principle Utah is now building into state policy.

Why it matters: If a machine is going to read your documentation, the agency that reads it first with the same rigor keeps the leverage.
What to do this week: Pull five charts ready to bill and confirm each one supports the visit frequency and skilled need billed. TrueTime or not, that check belongs inside your walls before submission.
06 Growth Tip
Team Select Cuts Hospitalizations 14.9% Using AI and Outcome Data
HOME HEALTH MODERATE

What happened: Team Select Home Care reported a 14.9% year-over-year reduction in patient hospitalizations using its own AI platform.

Why it matters: A hospitalization number you can prove is the strongest card you can bring to a value-based contract negotiation.
What to do this week: Pull your own 30-day hospitalization rate and decide whether it is a number you would put in front of a payer.
07 Retention Tip
Kaiser Owes Nurses Tens of Millions Over a Payroll System Error
ALL MODERATE

What happened: Kaiser Permanente will repay more than 1,000 nurses tens of millions of dollars in back wages caused by a clerical error in its payroll system.

Why it matters: Nothing erodes trust faster than paying people wrong, and per-visit pay models make those errors easy to hide and expensive to discover.
What to do this week: Pick three clinicians and trace one week of their visits to their paycheck, so you know your system can produce a wage trail per visit.
08 + 09 Case Snapshot / Case Outcome
What went wrong
The Denial Pattern Nobody Saw
HOME HEALTH MODERATE

What happened: Picture an agency that treats denials one at a time, appealing each as it arrives, while one payer quietly denies the same type of visit month after month.

Why it matters: Each denial looks like a one-off, but together they are a coding or documentation pattern that a payer's algorithm spotted long before the agency did.
What changed
Reading Denials as Data, Not Mail
HOME HEALTH MODERATE

What they did: The smarter path is to group every denial by payer, service, and reason each month, and fix the documentation behind the largest cluster first.

What happened: The appeals stop being a treadmill, because the agency fixes the pattern at the source instead of fighting the same denial claim by claim.

What to do this week: Sort your last 90 days of denials by reason code and find the one reason that shows up most often.
10 The Lighter Side
The Sandbox
ALL LOW

Utah is giving AI a sandbox to safely test prescription renewals with physician oversight and independent evaluation. Meanwhile, a hospice lost its Medicare billing over a typo on a renewal form. The robots got a sandbox. You got a revocation letter.
